Getting ready for an NDIS service referral

A good referral does not need to be perfect. It just needs enough information for the provider to understand the person, the goals and the urgency.

Helpful information to include

Include the participant's contact details, preferred communication method, NDIS number if available, plan dates, plan management type and the service being requested.

It also helps to include current supports, relevant diagnoses, safety concerns, access needs and whether interpreters or communication supports are required.

Describe the goal in plain language

Try to explain what the participant wants to be able to do, change or understand. A practical goal gives the provider a better starting point than a generic service label.

Examples might include building community confidence, reducing unsafe behaviours, preparing for work, improving home routines or supporting a transition into shared living.

Consent and contact

Make sure the participant or authorised representative understands the referral and agrees to be contacted.

List the best person for follow-up so the provider can quickly clarify missing details.
